Client Services Tools

This is just a starting point for client services tools, and most tools below are generic and very basic tools for small NGO's.  Also included is a client hoarding evaluation tool - the Environment Cleanliness and Clutter Scale. There are also some very useful person centred tools, an outcome planning tool designed particularly for people receiving Packages and tools for creating dementia friendly organisations.

  • Outcome Plan (for pdf click here) or (for word click here) - this tool has been developed by Jenny Bray Training and Consulting designed for people receiving Packages

  • Progress for providers - Checking your Progress in using Person-Centred Approaches (Managers) -  a self-assessment tool for front line managers to use individually and with their team.  Australian version developed by NDS and Helen Sanderson Associates
  • For some great Person Centred tools visit www.thinkaboutyourlife.org.  This site was developed by cancer survivors and people with chronic health conditions and has a great range of on-line tools such as 'one page profile', 'hopes and fears', 'good day, bad day', 'matching support', 'community map' etc.
  • Creating Dementia Friendly Communities Business Toolkit - developed by Alzheimer's Australia
  • Basic Service Request Form
  • Basic Care Plan and Agreement
  • Client Emergency Notification Form
  • Client File Audit Form
  • Environment Cleanliness and Clutter Scale - this form was developed by Graeme Halliday and John Snowdon and is very useful for a client hoarding evaluation

Tool Share Project Overview

To assist HACC and CCSP services embrace changes and reforms in the aged care and disability sector, Western Sydney Community Forum has developed Tool Share.  It provides an easy and free online method for encouraging and supporting HACC / CCSP services to share useful tools across the sector.

Templates, pro-formas, checklists and other tools in the following areas would be particularly useful:

- Implementing Consumer Directed Care

- Implementing individualised funding

- Wellness / Enabling approach

- Tools to assist with consumer choice and control

- Complying with Home Care Standards, Disability Service Standards and other Quality Assurance requirements

- Sample policies and procedures relevant to the reforms

- Business and corporate tools

- Specific tools for individual services (eg. Community Transport)

Western Sydney Community Forum is developing new tools and templates over the next few months and will be uploading them here and encourage the sector to forward any tools or templates that they have found useful to share.
